As per the notification issued by the EPFO department having reference No. Compliance/U/P78/2022/Advocacy/55643/13179, dated 7th Oct 2025, all the Establishments covered under the Employees Provident Fund Act are hereby directed to display the extracts of Form 5A mandatorily at the entrance of their establishment or on their website along with mobile application. The Establishments must abide by this notification and obey these directions within 15 days. Non-Compliance of the same will attract penal provisions as provided under this Act.

Form 5A Ingredients:

• i - Establishment Name & Address
• ii - Establishment Code No
• iii - Date of Commencement of Business
• iv - Rented or Owned
• v - Nature of Business
• vi - Occupier/Manager Details
